Scald the milk and butter in a pan and bring to a boil
Remove from heat and add flour
Stir vigorously

Heat the cream over medium heat until it thickens
Remove from heat and continue stirring until cooled slightly

Add eggs and beat until thickened

Grease a baking dish and add the dough, forming two cream puffs each 10cm in diameter
Bake for 45 minutes

Split the cream puffs in half and fill with caramel
Just before serving dust with confectioner's sugar.
