In a medium saucepan, combine the chocolate, heavy cream, whole milk, and salt

Heat over low heat, stirring constantly, until the chocolate melts and forms a smooth mixture

Bring to a simmer and let thicken slightly
Add vanilla extract and stir

Serve hot or cold with whipped cream, vanilla, chocolate, mint, or your preferred topping

If not using immediately, you can store the fondue in the refrigerator in an airtight container for up to a week
